| There is no formal application form. Requests for funding should be made by letter and the proposal should include the following information: | |
| 1. A description of the structure, purpose, history, and program of the organization including the names of key administrators and members of the board of directors. | |
| 2. A summary of the need for support and how it will be used. | |
| 3. Detailed financial data on the organization such as: | |

● An independent audit ● Annual operating budget ● Sources of income ● Breakdown of expenditures by program, administration and fundraising. ● A copy of IRS ruling, dated after 1969 classifying the organization as tax exempt under 501 (c) (3). |

| 4. A copy of the organizations most recent form 990. | |
| 5. A list of corporate donors and their level of support. | |
| 6. Other sources of funding. | |

7. Geographic Area/population served.
